Dependent Eligibility Documentation Requirements

As part of the dependent eligibility verification process for the medical, dental and vision plans, you will be required to submit supporting documentation to prove your relationship to each dependent.

Watch your Parkland email address and mailbox at your home address for details on the two-step dependent eligibility verification process. Do not submit documentation until Amwins sends you a verification request.

Step 1

Complete the online affidavit about your dependent relationships.

Step 2

Submit verification documents to confirm your responses.

If you don’t have Internet access, you can call the Eligibility Center at 866-691-6551 to provide your answers. Hours are Monday through Thursday, 7 a.m. to 6 p.m., and Friday, 7 a.m. to 4 p.m. Central time.

Need-To-Know Info

Watch for communication.

You will receive an initial and reminder email, both titled “Parkland Dependent Verification,” from mydependents.com (Amwins). You will also receive two letters and a phone call.

Pay attention to the deadline.

All communication will state the deadline for responding and the end date for dependent coverage.

Understand the importance of responding.

If you do not complete this process by the deadline, your dependent(s) will be dropped from coverage.

Spouse

Option 1: Copy of 2022 federal tax return showing “Married Filing Jointly” or “Married Filing Separately.”

Very Important!

  • Acceptable tax documentation is federal tax form 1040 or 1040A.
  • Please include only the first page of your 2022 tax return.
  • Mark out all financial information and the first five digits of all Social Security numbers.


Option 2: Copy of marriage certificate and proof of joint ownership
(mortgage or bank statement, property tax bill or rental/lease agreement)

  • Proof must include both employee’s name and spouse’s name.
Natural born child of employee up to age 26
  • Copy of child’s birth certificate
Stepchild
  • Copy of stepchild’s birth certificate, and
  • Copy of your marriage license
Adopted child
  • Copy of final adoption order, or
  • Copy of placement order approved by the court, or
  • Copy of birth certificate if your name is on it
Foster child or legal guardianship
  • Copy of court documents showing legal guardianship (with judge’s signature or court’s seal), and
  • Copy of your 2022 or 2023 (whichever is the most recent) federal tax return showing the child listed as your dependent. Mark out all financial information and the first five digits of all Social Security numbers.
